- Conduct initial evaluations to assess patients' needs and develop individualized treatment plans
- Develop and implement appropriate treatment plans based on patient needs and goals
- Monitor patient progress and adjust treatment plans as necessary
- Educate patients and caregivers on proper techniques to improve mobility, reduce pain, and prevent injuries
- Maintain accurate and timely documentation of patient progress
- Collaborate with other healthcare professionals to ensure patients receive comprehensive care
- Provide home health and/or outpatient services as needed, including supervisory visits, to ensure optimal outcomes
- A degree in Physical Therapy from an accredited program
- A Florida state PT license and current CPR/AED certification
- Home Health Experience preferred
- One-year experience in an outpatient setting preferred
- Ability to work independently and as part of a team
- Strong verbal and written communication skills
- Possess solid problem-solving skills and critical thinking skills
- Ability to work with a diverse patient population
